- Care Coordination & Health Navigation — Your care team meets monthly to review your progress and adjust your treatment to best meet your needs throughout your treatment. Healthcare navigators connect patients to referral partners, outside therapy, or other community services.
Patients keep the same medical provider and the same peer coach for the length of their care, rather than being handed between different faces at each visit.
